Patient Portal Information

In order to continually improve our patient experience, we sometimes need to make changes to bring you the best services and tools to participate in your patient care. Effective June 30, our current Patient Portal will be turned off to make way for a new portal.

The new portal will be through My Patient Visit. When you make an appointment, you should automatically receive an invitation to join the portal. If you do not receive this invite, please contact the office, and an invitation will be sent to you via text or email.

If you have an account with our existing portal, your username and password will not be valid on the new portal. You will need to create a new account.

General questions can be emailed to us using the Contact Us form. Please do not send personal health information through this link as it is not a secure means of communication like a Portal Secure Message would be.
